Qualität der pflegerischen Dokumentation und Auswirkungen auf die pflegerische Praxis – ein integratives Review

Zusammenfassung

Zielstellung

Das Ziel des integrativen Reviews war es, die verschiedenen Einflussfaktoren und Kriterien sowie Anforderungen an die Qualität von Dokumentationen sowie die Auswirkung, welche die Dokumentation auf die pflegerische Versorgungsqualität ausübt, zu erschließen.

Methodik

Es wurde eine Literaturrecherche in den Datenbanken Medline, CINAHL, Cochrane Library und Google Scholar sowie eine Handsuche durchgeführt. Der Zeitraum der Suche wurde auf die Jahre 2000 bis 2016 limitiert. Folgende beispielhafte Schlagwörter sowie weitere Abwandlungen wurden verwendet: „nursing, nursing record, nursing outcome“. Es wurden im Vorfeld zu jeder Fragestellung zwei übergeordnete Dimensionen respektive Kategorien deduktiv gebildet. Im Rahmen einer inhaltsanalytischen Auswertung der Studienergebnisse wurden induktiv Subdimensionen und Unterkategorien gebildet und anschließend den deduktiv gebildeten Dimensionen sowie Kategorien zugeordnet.

Ergebnisse

Bezogen auf die erste Fragestellung, wurden die übergeordneten Qualitätsdimensionen Inhalt und Methode identifiziert. Die Subdimensionen teilten sich auf in Pflegeprozess, strukturelle und externe Anforderungen, Personzentrierung, Plausibilität sowie Objektivität und Deutlichkeit. Bezüglich der Auswirkung der Dokumentation auf die pflegerische Versorgungsqualität wurden die Studien hinsichtlich der Auswirkungen auf die Patient*innen sowie das System ausgewertet. Hierbei konnte festgestellt werden, dass die Dokumentation womöglich Einfluss auf personelle Ressourcen nimmt und der Zeitaufwand für die Dokumentation abhängig vom Dokumentationssystem ist. Allerdings konnten deren Auswirkungen auf die pflegerische Praxis nicht eindeutig ermittelt werden.

Schlussfolgerungen

Infolgedessen erscheinen die Auswirkungen der Pflegedokumentation auf die Versorgungsqualität als nicht geklärt. Hier konnte lediglich ein Einfluss auf personelle Ressourcen und die Zeit festgestellt werden. Bezogen auf die Qualitätsaspekte der Dokumentation konnte eine Unterscheidung zwischen inhaltlichen sowie methodischen Qualitäten getroffen werden. Allerdings bleibt der Nutzen einer qualitativ hochwertigen Dokumentation unbeantwortet. Dementsprechend ist grundlegender Forschungsbedarf vonnöten.

Abstract

Objective

The aim of the integrative review was to identify the various influencing factors and criteria as well as requirements for the quality of documentation and the impact that documentation has on the quality of nursing care.

Methodology

A literature search in the databases Medline, CINAHL, Cochrane Library and Google Scholar as well as a manual search were carried out. The period of the search was limited to the years 2000–2016. The following exemplary keywords, nursing, nursing record, nursing outcome and other modifications were used. In advance, two superordinate dimensions or categories were deductively formed for each question. In the context of a content analytic evaluation of the study results, inductively subdimensions/subcategories were formed and subsequently assigned to the deductively formed dimensions or categories.

Results

In relation to the first question, the overarching quality dimensions of content and method were identified. The subdimensions were divided into care process, structural & external requirements, person-centeredness, plausibility and objectivity & clarity. With regard to the impact of the documentation on the quality of nursing care, the studies were evaluated with regard to their effects on patients and the system. It was found that the documentation may have an influence on personnel resources and that the time required for documentation depends on the documentation system. However, their effects on nursing practice could not be clearly determined.

Conclusions

As a result, the effects of nursing documentation on the quality of care do not appear to have been clarified. Here only an influence on human resources and time could be determined. With regard to the quality aspects of the documentation, a distinction could be made between content-related and methodological qualities. However, the benefits of high-quality documentation remain unanswered. Accordingly, fundamental research is needed.
