„SPERMAGGEDON“ ODER: VON DER WICHTIGKEIT, DEN MANN BEI UNERFÜLLTEM KINDERWUNSCH MITZUBEHANDELN

Zusammenfassung

Bereits jedes 6. Paar in Österreich ist ungewollt kinderlos. Das heißt, dass selbst nach 1–2 Jahren regelmäßigem, ungeschütztem Geschlechtsverkehr keine Schwangerschaft eintritt.

Aufgrund dieser Tatsache können Österreichs Kinderwunschinstitute einen gewaltigen Zulauf verzeichnen. Statistisch gesehen sitzt in jeder Schulklasse bereits mindestens ein Kind, das mittels assistierter Reproduktionstechniken (IVF, ICSI usw.) gezeugt wurde. Sucht man nach den Ursachen für den unerfüllten Kinderwunsch eines Paares, so liegen 40 % der Ursachen bei der Frau und ebenfalls 40 % beim Mann.

Obwohl der Anteil der männlichen Ursachen für ungewollte Kinderlosigkeit gleich hoch ist wie der weibliche (40 %), wird die Unterstützung und Therapie der männlichen Subfertilität, der sogenannte „male factor“, schlichtweg übersehen. Und das, obwohl sich die Qualität der Spermien in den letzten 30 Jahren um 50 % verschlechtert hat. So sehr, dass die WHO 2010 eine Korrektur/Adaption der Normwerte vornahm, sodass überhaupt noch unauffällige Ergebnisse von Spermiogrammen möglich sind.

Zudem spielt der „male factor“ eine wesentliche Rolle als Ursache für Frühaborte und sollte schon allein deshalb zur Abortprophylaxe mitbehandelt werden.

In diesem Artikel breche ich somit eine Lanze dafür, bei Kinderwunschpaaren, die in die TCM-Praxis kommen, künftig in jedem Fall den Mann mitzubehandeln.

Summary

On average one out of 6 Austrian couples experience infertility, defined as one or two year of unsuccessful attempts of trying to conceive a baby. Due to this fact, the Austrian fertility institutes’ popularity is increasing. Talking statistically, in every Austrian classroom there is at least one pupil that was fathered with the help of assisted reproduction. Looking at the reasons of unfulfilled parenthood, 40 % are female issues and 40 % are male issues.

Interestingly our society as well as our medicine focus quite exclusively on treating the female part of the childless couple. Thus the percentage of male issues for infertility — the so called male factor — is the same as the female one, treating the male partner is very under-represented, despite the fact that the seminal quality has declined within the last 30 years by 50 %. It downgraded so dramatically that the WHO had to correct and adapt the standard value for the semen analyses in 2010 to make sure that one could get non-pathological semen analyses results at all. On top of that the male factor plays an important role in early pregnancy loss and should be treated in any case to prevent abortion.

In this article I’d like to stand up for a obligatory treatment of the male factor as well, whenever an infertile couple enters the Chinese Medicine office in future.
