Vermutete Überempfindlichkeitsreaktionen auf Arzneimittel sollen diagnostisch geklärt werden, um einerseits schwere Reaktionen bei erneuter Exposition, andererseits ungerechtfertigte Einschränkungen der Medikation zu vermeiden. Hauttests sind in diesem Zusammenhang wichtige Testverfahren, um Sensibilisierungen zu erfassen. Es werden aktuell die Methoden des European Network on Drug Allergy (ENDA) empfohlen, die in verschiedenen Leitlinien und Positionspapieren festgehalten wurden. Für die Testung der Betalaktamallergie hat sich 2003 in diesen Empfehlungen ein Fehler bezüglich der angegebenen Testkonzentrationen für die zwei zu der Zeit in Europa erhältlichen kommerziellen Penizillin-Testkits eingeschlichen [1], der auf mehrere Positionspapiere [2, 3, 4] und auch auf die oben genannte Arbeit [5] übertragen wurde. In diesen Publikationen wurden die Konzentrationen fälschlicherweise als mmol/L anstatt mol/L wiedergegeben. Bezüglich der kommerziell verfügbaren Testkits hat ein Hersteller die Produktion der Testkits eingestellt (Allergopen, Allergopharma, Germany). Der andere Testkit enthält seit 2012 eine höher aufgereinigte Zusammensetzung des „Major Determinants“ Benzylpenicilloyl-octa-L-lysin statt Benzylpenicilloyl-poly-L-lysin und verbesserte Stabilität von Natrium-Benzylpenilloat als einziges „Minor Determinant“ anstatt der Minor Determinant Mixture (DAP® Kit, Diater, Madrid, Spanien). Dieses ist der nun einzige kommerziell in Europa erhältliche Testkit.
Die Korrektur betrifft das sogenannte „Major Determinant“ (Benzylpenicilloyl-octa-L-lysin) and die „Minor Determinant“ (Natrium-Benzylpenilloat). Es befinden sich nach Rekonstruktion mit 1 ml Lösungsmittel je 8,6 × 10-5 mol/L (bezogen auf acht Benzylpenicilloyl-Moleküle) beziehungsweise 1.5 × 10-3 mol/L (Natrium-Benzylpenilloat) und nicht wie in den Übersichtsartikeln angegeben 5 × 10-5 mM Penicilloy-Poly-l-lysine beziehungsweise 2 × 10-2 mM Minor Determinant Mixture. Diese Angaben in Tab. 4 sind dementsprechend zu korrigieren. Wenn die dem DAP® Testkit beiliegenden schriftlichen Anleitungen eingehalten werden, ergeben sich automatisch die richtigen Testkonzentrationen.
