Samenvatting
Dertig jaar geleden werd aangetoond dat de inname van foliumzuur vóór conceptie en in het begin van de zwangerschap het risico op neuraalbuisdefecten (neural tube defects (NTD)) aanzienlijk vermindert. Ondanks gezondheidspromotie rond foliumzuursupplementen in Europa is de prevalentie van NTD’s niet wezenlijk gedaald. In de Verenigde Staten (VS) daarentegen werd een grote terugval van NTD’s geobserveerd dankzij de verplichte verrijking van bloem met foliumzuur. Een werkgroep onder leiding van prof.dr. Joan Morris van de St. George’s universiteit van Londen onderzocht hoeveel NTD-zwangerschappen vermeden hadden kunnen worden indien Europa sinds 1998 systematische foliumzuurverrijking toegepast zou hebben, analoog aan de VS. De berekeningen gebeurden op basis van 35 EUROCAT-registers met gegevens over aangeboren afwijkingen, aangevuld met data uit de literatuur over serumfolaatgehaltes en inname van foliumzuursupplementen op populatieniveau. Tussen 1998 en 2017 werd het aantal zwangerschappen met een NTD in Europa geschat op 95.213, en dit op 104 miljoen geboorten in 28 landen, wat resulteert in een prevalentie van 0,92 per 1.000 geboorten. Het aantal NTD-zwangerschappen dat over een periode van 20 jaar had kunnen worden vermeden indien de 28 Europese landen van de EU in 1998 gestart zouden zijn met foliumzuurverrijking van bloem, wordt geschat tussen de 14.600 en 19.500. In België zou dit neerkomen op 270 tot 370 vermeden NTD-zwangerschappen op 20 jaar tijd. Dit onderzoek berekende dat het niet implementeren van foliumzuurverrijking van bloem in Europa jaarlijks kan leiden tot neuraalbuisdefecten bij bijna 1.000 zwangerschappen.
