Hintergrund
Die Diagnose seltener Erkrankungen erfolgt häufig verzögert. Grund dafür ist ein Mangel an spezifischem Krankheitswissen und Krankheitsbewusstsein angesichts der Vielzahl der Erkrankungen.
Fragestellung
Wie werden seltene Erkrankungen diagnostiziert? Können typische diagnostische Fehler in Fällen seltener Erkrankungen vermieden werden?
Material und Methode
Der Weg zur Diagnose einer seltenen Erkrankung soll beschrieben werden.
Ergebnisse
Notwendiger Ausgangspunkt einer korrekten Diagnose einer seltenen Erkrankung ist das initiale Verdachtsmoment bei Patienten mit erfolgloser „diagnostischer Odyssee“, atypischem Krankheitsverlauf oder fehlendem Therapieansprechen. Anschließen sollten sich eine komplette Fallaufarbeitung inklusive unvoreingenommener Anamnese sowie eine fokussierte und vollständige Diagnostik. Spezifische Untersuchungen können helfen, die Diagnose einzugrenzen oder zu sichern. Der Einsatz klinischer Entscheidungsunterstützungssysteme für seltene Erkrankungen sollte evaluiert und weiterentwickelt werden.
Schlussfolgerungen
Die initiale Verdachtsstellung und anschließende fokussierte und vollständige Fallaufarbeitung sind der Schlüssel zur Diagnose einer seltenen Erkrankung.
Abstract
Background
The diagnosis of rare diseases is often delayed due to a lack of specific knowledge and awareness due to the large number of diseases.
Objective
How are rare diseases diagnosed? Can typical diagnostic errors in rare diseases cases be avoided?
Method
The path to the diagnosis of a rare disease is described.
Results
The necessary starting point for the correct diagnosis of a rare disease is the initial degree of suspicion in patients on an unsuccessful “diagnostic odyssey”, with an atypical disease progression or a lack of response to therapy. This should prompt a complete case revision that includes an unbiased re-examination of the patient history. It should be followed by a focused and comprehensive diagnostic process. Specific investigations can help to specify or confirm a diagnosis. The use of clinical decision support systems for rare diseases should be evaluated and further developed.
Conclusion
The initial degree of suspicion and the subsequent focused and comprehensive diagnostic processes are essential for the diagnosis of rare disease.
