Sommario
Le fratture dell’estremo prossimale di omero sono relativamente frequenti e rappresentano il 4–6% di tutte le fratture, con una maggiore prevalenza nella popolazione anziana (infatti, il 75% del totale coinvolgono pazienti over-50). Il loro trattamento continua a essere oggetto di discussione, non solo nella scelta tra l’approccio incruento (che può condurre a buoni risultati con rischi moderati) e quello cruento, ma soprattutto nella selezione di un metodo chirurgico piuttosto che un altro fra i tanti proposti. In una moderna ottica di cura, volta ad assicurare un’adeguata qualità di vita durante il trattamento e un rapido reinserimento sociale del paziente traumatizzato, i limiti del trattamento conservativo non possono essere ignorati e, negli anni, si è consolidato un indirizzo sempre più decisamente chirurgico nel trattamento delle fratture. Questo studio nasce dall’esigenza di conciliare, nel trattamento di queste fratture, il miglior recupero funzionale (data la complessa struttura anatomica dell’articolazione “spalla”), con la minore incidenza di complicanze relative non solo all’intervento chirurgico e al periodo di degenza ospedaliera, ma anche alle sequele psicologiche che il trauma e il trattamento stesso possono avere sui pazienti più spesso interessati da questa lesione ovvero i pazienti anziani. Riportiamo i risultati di 33 pazienti (20 donne e 13 uomini con età media di 76 anni) con fratture dell’epifisi prossimale di omero ottenuti con l’utilizzo dell’osteosintesi elastica con chiodini di Galluccio. Al follow-up a 12 mesi abbiamo ottenuto complessivamente un buon recupero funzionale con un valore medio di Constant score di 80,9. La sintesi elastica con chiodi di Galluccio può quindi essere considerata una valida alternativa alle metodiche classiche esistenti e una tecnica elettiva nelle fratture di epifisi prossimale di omero nel soggetto anziano.
Abstract
Fractures of the proximal humerus are relatively common and represent 4–6% of all fractures, with a higher prevalence in the elderly population (in fact, 75% of the total involving patients over-50). Their treatment continues to be debated, not only in the choice between nonoperative approach (that allows satisfactory results with moderate risk) and surgical option, but especially in the selection of one surgical method rather than another among the many proposed. In a modern viewpoint of care aimed at ensuring adequate quality of life during treatment and social reintegration of the trauma patient, the limits of conservative treatment cannot be ignored and, over the years, has established a surgical address in the treatment of fractures more decisively. This study arises from the need to reconcile, in the treatment of these fractures, the best functional recovery, with the lowest incidence of complications related to surgery and not, but also to the psychological sequelae that trauma and the treatment itself can give to the elderly patients, most often affected by this injury.
We report the results of 33 patients (20 women and 13 men, mean age 76 years) with fractures of the proximal humerus obtained using elastic nails osteosynthesis of Galluccio. At a 12 months follow-up, we have achieved an overall good functional recovery with a mean Constant score of 80.9. The synthesis of elastic nails named Galluccio can therefore be considered a viable alternative to more invasive methods in fractures of the proximal epiphysis of the humerus in the elderly.
