Zusammenfassung
Die Polkörperdiagnostik (PKD) ermöglicht die zyto- und molekulargenetische Untersuchung von Eizellen. Das Haupteinsatzgebiet der PKD ist der Nachweis von Chromosomenfehlverteilungen in Eizellen. In der vorliegenden Arbeit stellen wir unsere Untersuchungen nach Einsatz der Polkörperdiagnostik bei bekannter zytogenetischer Prädisposition der Frau vor. Unsere Daten belegen, dass bei mütterlichen Translokationen die Durchführung der Polkörperdiagnostik durchaus sinnvoll ist, da ein Großteil der Eizellen eine Fehlverteilung der an der jeweiligen Translokation beteiligten Chromosomen aufweist. Der frühzeitige Ausschluss von Eizellen mit einer vererbten unbalancierten Translokation resultiert in akzeptablen Schwangerschafts- und Geburtenraten. Interessant sind unsere Befunde bei Frauen mit niedriggradigen Gonosomenmosaiken in peripheren Lymphozyten. Gonosomale Aneuploidien sind auch bei diesen Patientinnen an der Gesamtzahl der Chromosomenfehlverteilungen in reifen Eizellen nicht übermäßig häufig beteiligt.
Abstract
Polar body diagnosis (PBD) enables the indirect cyto- and molecular genetic analysis of oocytes. Its main application is the detection of chromosomal aneuploidies in oocytes. In this article, we present our investigations on the use of PBD in cases with a known cytogenetic predisposition. Our data show that the application of PBD is beneficial for maternal translocations as the majority of oocytes are unbalanced for the chromosomes involved in the translocation. The exclusion of aneuploid oocytes results in acceptable pregnancy and delivery rates. Our data on women with low level sex chromosome mosaicism in peripheral lymphocytes are equally interesting. Gonosomal aneuploidies were not found to contribute predominantly to the chromosomal disorders identified in mature oocytes in these patients.
