Zusammenfassung
In der vorliegenden Arbeit wurde die prognostische Bedeutung des Plasminogenaktivators vom Urokinasetyp (uPA) und des Plasminogenaktivator-Inhibitors (PAI-1) beim Plattenepithelkarzinom der Mundhöhle untersucht. Von 58 primär operierten Patienten mit einem Plattenepithelkarzinom der Mundhöhle war Gewebe aus dem Tumorzentrum entnommen worden. Mit der ELISA-Technik konnte der Gehalt von uPA und PAI-1 in den Zellen bestimmt werden.
Die statistische Auswertung erfolgte mit dem χ2-Test und dem Log-Rank-Test.
Die mittlere Nachbeobachtungszeit (n=58) lag bei 23 Monaten. Zur Beurteilung der prognostischen Bedeutung wurde die rezidivfreie Zeit herangezogen. Die mittlere rezidivfreie Zeit lag bei 19 Monaten. 23 (40%) Patienten erkrankten an einem Rezidiv (Lokalrezidiv n=13, Lymphknotenrezidiv n=3, Lokal- und Lymphknotenrezidiv n=1, Lymphknoten- und Hautmetastase n=1, Fernmetastase n=5). Bei Patienten mit einem Rezidiv ist uPA (p<0,012) und PAI-1 (p<0,014) bereits im Primärtumor signifikant erhöht. Es zeigt sich, dass Patienten mit postoperativer Nachbestrahlung häufiger ein Rezidiv bekommen (n.s.).
Mit dem multiplen Log-Rank-Test wurde für uPA/PAI-1 ein optimierter Schwellenwert (Cut-off-level) ermittelt.
Für uPA beträgt der Cut-off-level 4,58 ng/mg, für PAI-1 106,3 ng/mg. Es besteht keine signifikante Korrelation für Patienten mit niedrigen, d. h. Werten kleiner als der Cut-off-Level bzw. hohen Werten, d. h. Werten größer als der Cut-off-Level, zu den herkömmlichen klinischen Parametern, wie Lokalisation, N-Stadium, T-Stadium, Differenzierung, und Residualtumorstatus. Bei älteren Patienten (>58 Jahre) liegen signifikant häufiger hohe uPA- und PAI-1-Werte vor (p<0,017 für uPA, p<0,02 für PAI-1). Für das rezidivfreie Überleben, dargestellt mit Kaplan-Meier-Kurven, zeigen sich signifikant häufiger Rezidive bei Patienten mit hohen Werten (p<0,009 für uPA, p<0,008 für PAI-1). Bei einer Aufteilung des Patientenguts in 3 Gruppen (Gruppe 1: uPA und PAI-1 niedrig (n=35), Gruppe 2: uPA oder PAI-1 hoch (n=12), Gruppe 3: uPA und PAI-1 hoch (n=11)) traten Rezidive signifikant häufiger in Gruppe 3 als in Gruppe 1 und 2 auf (p<0,023).
Um die postoperative Nachbehandlung in der Auswertung zu berücksichtigen, wurden die Patienten mit ausschließlich operativer Therapie (n=29) und mit postoperativer Radiatio (n=29) gesondert untersucht. Für beide Gruppen wurden jeweils Cut-off-Levels berechnet. In der Gruppe der ausschließlich operativ behandelten Patienten ist der Cut-off-Level für uPA 5,63 ng/mg, für PAI-1 106,3 ng/mg. In der Gruppe der Patienten, die postoperativ bestrahlt wurden, ist der Cut-off-Level für uPA 4,13 ng/mg, für PAI-1 97,02 ng/mg. Bei der Darstellung der Kaplan-Meier-Kurven zeigt sich eine deutliche Tendenz, dass Patienten mit höheren Werten häufiger an einem Rezdiv erkranken. Signifikant ist dies in der Gruppe der ausschließlich operierten Patienten für PAI-1 (p<0,01) und in der Gruppe der nachbestrahlten Patienten für uPA (p<0,04).
Abstract
This article examines the prognostic importance of urinase type plasminogen activators (uPA) and of plasminogen activator inhibitors (PAI-1) in cases of primary oral squamous cell carcinoma. Tissue samples were taken from the core of the tumour in 58 such primary surgical patients. The levels of uPA and PAI-1 were determined using ELISA.
Statistical significance was calculated using the χ2- and log-rank tests.
The mean follow-up (n=58) was 23 months. In order to determine prognostic value, the time before relapse was used. The mean time before relapse was 19 months. A total of 28 (40%) patients relapsed (local n=13, lymph node n=3, local and lymph node n=1, lymph node and skin n=1, other locations n=5). Such patients had significantly raised uPA (P<0.012) and PAI-1 (P<0.014) levels in the primary tumour.
A optimal cutoff value for uPA (4.58 ng/mg) and PAI-1 (106.3 ng/mg) was determined using the multiple log-rank test.
There was no significant correlation for patients with low or high levels (i.e. lower or higher than the cutoff value, respectively) with the usual clinical parameters such as localisation, N-stage, T-stage, differentiation and residual tumor status. Older patients (>58 years) had significantly higher levels of uPA and PAI-1 (P<0.017 and P<0.02, respectively). The likelihood of relapse was significantly higher in patients with high levels (uPA P<0.009, PAI-1 P<0.008). If the patients were divided into three groups depending on uPA and PAI-1 levels (group 1: uPA and PAI-1 low, n=35; group 2: uPA or PAI-1 high, n=12; group 3: uPA and PAI-1 high, n=11), relapses were more common in group 3 than in groups 1 or 2 (P<0.023).
Patients with only surgical therapy (n=29) and those with postoperative radiotherapy (n=29) were used to evaluate postoperative follow-up. Cutoff levels were calculated for both groups. In the surgical therapy group this was uPA=5.63ng/mg and PAI-1=106.3 ng/ml and in the surgical therapy plus radiotherapy group uPA=4.13 ng/mg and PAI-1=97.02 ng/mg. Kaplan-Meier curves showed a marked tendency for patients with higher levels to relapse more often. This is significant for surgical patients for PAI-1 (P<0.01) and for radiotherapy patients for uPA (P<0.04)
