Zusammenfassung
Chronische Spasmen als Extremform einer Tonusänderung sind höchstwahrscheinlich nicht — wie im Schmerz-Spasmus-Schmerz-Konzept angenommen — von einer Schmerzquelle im verspannten Muskel selbst bedingt. Die wirklichen Ursachen für die Verspannung müssen außerhalb des Muskels gesucht werden. Eine Behandlung der Muskelverspannung darf daher nicht auf den verspannten Muskel beschränkt sein, sondern der Therapeut sollte gezielt nach Schmerzquellen in anderen Muskeln oder benachbarten Gelenken suchen. Die neuroplastischen Veränderungen, die sich im Rückenmark als Reaktion auf eine schmerzhafte Muskelläsion ausbilden, überdauern den nozizeptiven Impulseinstrom aus dem Muskel bei weitem und brauchen Zeit, um sich zurückzubilden. Um die Sensibilisierung des Zentralnervensystems zu verhindern, ist eine möglichst frühzeitige und effektive Therapie der peripheren Schmerzquelle von größter Wichtigkeit.
Abstract
Chronic spasms as the extreme form of a charge in tonicity most likely are not caused by a source of pain in the tense muscle itself — which is an assumption made in the pain-spasm-pain concept. The true causes of the tension must therefore be sought outside of the muscle. For this reason, the treatment of a tense muscle must not be restricted to the tense muscle only, but rather the therapist should search for sources of pain in other muscles or neighbouring joints. The neuroplastic changes manifesting in the bone marrow in response to a pain-associated muscle lesion persist much longer than the influx of nociceptive impulses from the muscle and need sufficient time to regress. In order to prevent sensitization of the central nervous system, early and effective therapy of the peripheral source of pain is of utmost significance.
