Zusammenfassung
Einleitung
Die Rehabilitation nach Multiligamentverletzungen stellt nach wie vor eine Herausforderung für den Arzt, Physiotherapeuten und Patienten dar. In der gängigen Literatur gibt es hierfür nur wenig Evidenz. Insbesondere aufgrund der Verletzungsschwere sollte ein strukturiertes Konzept und eine entsprechende Aufklärung des Patienten angestrebt werden.
Methoden
Die Nachbehandlung wird grundsätzlich in 4 Hauptphasen unterteilt, welche je einen unterschiedlichen Behandlungsschwerpunkt haben. Phase I (1.–6. postoperative Woche): maximale Protektion der Bandrekonstruktion, Phase II (7.–12. postoperative Woche): moderate Protektion der Bandrekonstruktion und Mobilisation, Phase III (13.–24. postoperative Woche): muskuläre Kräftigung, Steigerung des Bewegungsumfangs, Phase IV (ab 24. postoperative Woche): Wiederaufnahme der sportlichen Aktivität. Entscheidend ist es, für eine Balance zwischen Wiederherstellung der Funktion und Beweglichkeit des Kniegelenks und der Stabilität zu sorgen, ohne hierbei die Bandheilung zu gefährden.
Schlussfolgerung
Die Rekonstruktion von Multiligamentverletzungen ist nicht nur operativ anspruchsvoll. Ein konsequentes Nachbehandlungsregime und die Compliance des Patienten sind erforderlich, um die Bandheilung zu gewährleisten. Hierbei spielt die Kommunikation zwischen Patient, Physiotherapeut und Arzt eine entscheidende Rolle.
Abstract
Introduction
Rehabilitation after multiligament injuries still poses a challenge for physicians, physiotherapists and patients. In the current literature, there is limited evidence for the correct postoperative rehabilitation. A structured rehabilitation concept and appropriate informative clarification for the patient are necessary, especially because of the severity of injury.
Methods
In principle, the follow-up treatment is divided into four main stages with different focal points: stage I (postoperative weeks 1–6) maximum protection of the ligament reconstruction, stage II (postoperative weeks 7–12) moderate protection of the reconstruction + mobilization, stage III (postoperative weeks 13–24) strengthening of muscles and increased extent of mobilization and stage IV (postoperative week 24 onwards) return to sports activities. The decisive factor is the balance between recovery of function and movement of the knee and stability without compromising ligament healing.
Conclusion
The reconstruction of multiligament injuries is not only a surgical challenge. To ensure ligament healing a consistent rehabilitation program and patient compliance are necessary; therefore, the permanent dialog between patient, physiotherapist and physician plays a decisive role.
