Summary
Background: Primary intestinal non-Hodgkin lymphoma is a specific entity of intestinal diseases. 58% of intestinal non-Hodgkin lymphomas represent as intestinal emergencies.
Methods: In a prospective non-randomized study epidemiology, diagnosis, histopathology and surgical therapy of intestinal non-Hodgkin lymphomas were investigated.
Results: 26 intestinal non-Hodgkin lymphomas were operated according to the protocol before studied histopathological examination. 14 highly malignant, 5 low malignant non-Hodgkin lymphomas, 4 undefined low/highly malignant and 3 others were observed. In 15 of 26 patients (58%) the diagnosis non-Hodgkin lymphoma was known only after emergency operation. 11 of 26 patients (42%) were known preoperatively by ultrasound, endosonography, intestinoscopy, enema of Sellink, CT-scan of chest, abdomen and pelvic or bone-marrow puncture as intestinal non-Hodgkin lymphomas which required extensive staging laparotomies: regional and juxtaregional lymphatic sampling, liver biopsie and splenectomy if involved.
Conclusions: The high incidence of emergency laparotomies is responsible for an inadequate lymphoma surgery. Extensive surgery for intestinal non-Hodgkin lymphoma is indicated mainly for staging. Treatment is managed by different modalities: operation — radiotherapy — chemotherapy.
