Zusammenfassung
Störungen des Hormonhaushaltes, des Kalziumhaushaltes und des Natriumhaushaltes erzeugen oft enzephalopathische Syndrome, die teilweise (endokrine Dysfunktionen) störungsspezifisch ausgeprägt sind. Kaliumentgleisungen induzieren keine zentralen Störungen, jedoch kardiale Arrhythmien und neuromuskuläre Ausfälle bis hin zur Tetraparese. Abweichungen im Kalzium-, Magnesium- und Phosphathaushalt dagegen können Symptome des zentralen und des peripheren Nervensystems sowie Herzrhythmus¬störungen bewirken. Komplikationen dieser Enzephalopathien sind durch Hirnödem, Krampfanfälle, aber auch durch stauungsbedingte Mikroblutungen bei zu starker Entwässerung des ZNS zu beobachten. Therapeutisch steht der Ausgleich der metabolischen Dysregulation im Vordergrund, wobei differenzierte Empfehlungen sowohl für sehr rasche Therapie (z.B. bei akutem Kortikosteroidmangel, hyperthyreoten Koma) als auch gegen rasche Ausgleichsversuche (bei chronischer Hyponatriämie, Gefahr der osmotischen Demyelinisierung!) beachtet werden müssen. Potenziell schädliche Überkorrekturen sollen durch ein konsequentes Monitoring der Laborwerte und der klinischen Befunde vermieden werden.
