Introduction
An understanding of the pathophysiology of migraine should be based upon the anatomy and physiology of the pain-producing structures of the cranium integrated with knowledge of central nervous system modulation of these pathways. Headache in general, and in particular migraine (Goadsby et al. 2002) and cluster headache (Goadsby 2002b), is better understood now than has been the case for the last four millennia (Lance and Goadsby 2005). This entry will set out the current understanding of migraine.

Migraine is in essence a familial episodic disorder whose key marker is headache with certain associated features (Table 1). It is these features that give clues to its migraine pathophysiology and will ultimately provide insights leading to new treatments.
