Contexte
Peu de données sont disponibles sur la pratique de la coloscopie et son rendement diagnostique en Europe. Nous avons mené une étude prospective nationale d’évaluation de la pratique de la coloscopie et des performances diagnostiques de cet examen.
Méthodes
Un questionnaire en ligne a été envoyé à 2 600 gastroentérologues français. Les informations sur les professionnels de santé ayant répondu ont été recueillies et comparées aux données nationales de l’ordre des médecins. Durant une semaine, les données de toutes les endoscopies digestives réalisées, puis secondairement les résultats histologiques étaient renseignés en ligne. Les résultats ont été extrapolés à une année entière d’endoscopie en France, et les facteurs associés au taux de détection d’adénome (TDA), de polype ou de cancer colorectal (CCR) ont été déterminés.
Résultats
Trois cent quarante-deux gastroentérologues représentatifs de la spécialité ont fourni des données sur 3 266 coloscopies. La coloscopie était complète dans 90 % des cas. Les patients étaient des hommes dans 48 % des cas, âgés de plus de 50 ans dans 77 % des cas. Les indications de la coloscopie étaient le dépistage du CCR dans 40 % des cas, des symptômes digestifs dans 39 % des cas, et un test Hémoccult® positif dans 5 % des cas. Une résection de polype était réalisée dans 31,3 % des cas, et les lésions étaient supérieures à 10 mm dans 21 % des cas, n supérieur à 3 dans 14 % des cas, et de localisation colique droite dans 47 % des cas. Les taux de détection d’adénome et d’adénocarcinome étaient de 17,7 et 2,9 % des cas, respectivement. Les facteurs associés à un TDA élevé étaient le sexe masculin (p < 0,0001), un âge supérieur à 50 ans (p < 0,0001), les facteurs de risque familiaux de CCR (p = 0,0002), un antécédent personnel de polype ou de CCR (p < 0,001) et un test Hémoccult® positif (p = 0,0008). Le taux de CCR était trois fois plus élevé chez les patients n’ayant jamais bénéficié d’une coloscopie (4,2 vs 1,4 %, p < 0,0001).
Conclusion
Ces données nationales prospectives sur la pratique de la coloscopie, incluant pour la première fois en France des résultats histologiques, soulignent l’importance de l’amélioration de la préparation colique, retrouvent un TDA global de 17,7 % et confirment le rôle préventif de la surveillance coloscopique sur la survenue du CCR.
Abstract
Context
There is little data available on colonoscopy practices and its diagnostic performance in Europe. We have carried out a national prospective study, evaluating colonoscopy practices and the diagnostic performance of this examination.
Method
An online questionnaire was sent to 2,600 French gastroenterologists. Information from healthcare professionals who responded was collected and compared with national data from the Ordre des Médecins (French equivalent of the General Medical Council in the UK). Over a one week period, data on all gastrointestinal endoscopies carried out, including histological results, were recorded online. The results were extrapolated for a whole year of endoscopy in France, and factors linked to adenoma detection rates (ADR), polyps or colorectal cancer (CRC) were determined.
Results
342 gastroenterologists representing the speciality provided data on 3,266 colonoscopies. In 90% of cases, this was a full colonoscopy. 48% percent of patients were male, aged over 50 years in 77% of cases. Colonoscopy was indicated to investigate colorectal cancer in 40% of cases, gastrointestinal symptoms in 39% of cases and following a positive Hemoccult® test in 5% of cases. Polyp resection was performed in 31.3% of cases, with lesions being > 10 mm in 21% of cases, n > 3 in 14% of cases and occurring in the right colon in 47% of cases. Adenoma and adenocarcinoma detection rates were 17.7% and 2.9% of cases respectively. Factors associated with an increased ADR were: being male (p < 0.0001), being > 50 years old (p < 0.0001), family risk factors of colorectal cancer (p = 0.0002), a previous personal history of polyps or colorectal cancer (p < 0.001) and a positive Hemoccult® test (p = 0.0008). CCR rates were three times higher for patients who had never had a colonoscopy before (4.2% vs 1.4%, p < 0.0001).
Conclusion
This prospective national data on colonoscopy practices, including for the first time in France histological results, underlines the importance of improvements to colon preparation, resulting in an overall ADR of 17.7% and confirming the preventive role of colonoscopic monitoring in the appearance of CRC.
